The successful candidate will utilise the HEAPS system(AI predicatable model) in co-ordination of clinical programs in Disease Risk & Care coach Management experience in building effective relationships with our members and manage the schemes risk.
, Requirements,
- Grade 12 or
- Tertiary degree
- If grade 12, minimum of 2 years for call centre experience required
- Computer literacy (MS Office)
- Disease risk management experience
- Soft skills and Call centre experience
- Engage with the patients over various touchpoint calls to understand their current medical health
- status, symptoms, past medical/surgical history, medications taken, labs done etc.
- Conduct system scheduled and directed outbound calls
- To enrol new patients into the system
- Use system prompts to assess clinical risk for admissions and re-admission
- Use system prompts to document progress of their registered health condition, access to treatment plans and compliance to treatment
- Use system prompts to coordinate access to existing benefits where members qualify
- To handle patient queries and providing response from pre-defined FAQs.
- To receive regular feedback from team managers and undergo program related training
- and on the job training on a regular basis.
