Linkage to Care Officer X 4 (Onsite)

THINK is looking for Linkage to Care Officers X 4 in Bloemfontein and Sasolburg. Are you passionate about improving public health outcomes? Duties And Responsibilities Include But Not Limited To
  • Ensure that all processes related to patient linkage to TB care are streamlined and effective. Identify potential gaps and create strategies for efficient patient care continuity.
  • Organise and conduct training sessions for the team on the latest data management techniques, tools, and best practices, ensuring everyone is updated on the recent changes in TB data management.
  • Handle large volumes of data related to TB cases, ensuring accurate and timely input into the system. Review data entries for errors, rectify any inconsistencies, and maintain data integrity.
  • Continuously assess and review the effectiveness of implemented data management strategies. Ensure that the linkage and contact processes are up to date with the latest healthcare protocols.
  • Participate in regular meetings with facility management and staff. Provide updates on data management efforts, gather feedback, and implement changes where necessary.
  • Foster open lines of communication with doctors, nurses, lab technicians, and other healthcare professionals. Address any concerns or queries related to TB data and linkage.
  • Collaborate with healthcare facilities and outreach teams, ensuring that patients diagnosed with TB are quickly linked to appropriate treatment and care. Address any barriers that might hinder this process.
  • Innovate and implement efficient communication systems to enhance collaboration between different departments, facilities, and outreach teams. This might include the introduction of new communication tools or platforms.
  • Continuously upskill by attending relevant training sessions, workshops, and seminars. Stay updated with the latest in TB care and data management.
Requirements
  • A Grade 12-Matric Certificate or equivalent.
  • Certificate in Health Sciences, or Data Management is preferred.
  • Minimum of 2 years' experience in data management, preferably within a health or clinical setting.
  • Prior experience with NHLS data or any other health-related data systems is an advantage.
  • Previous involvement in TB program implementation is desirable.
  • Experience in patient linkage systems or healthcare coordination is a plus.
  • Proficiency in Microsoft Office, especially advanced Excel and Word functionalities.
  • Familiarity with health information systems (HIS) or similar database management tools.
  • Ability to understand and interpret medical and health data accurately.
